This high-engagement program is designed for employees with chronic or high-risk conditions such as heart disease, diabetes, and stroke. Each participant completes a wellness assessment with a dedicated nurse advisor, who develops a personalized treatment plan. Monthly check-ins monitor vital signs, weight, and lifestyle progress, while setting new goals to encourage continued improvement. Employees who meet their goals may receive wellness incentives, and enrollment bonuses can further encourage participation. Tailored educational materials and reminders help employees stay on track and adhere to their treatment plans.

Foundational Wellness Support
This lighter-touch program provides an initial wellness assessment and educational resources to help employees make positive lifestyle changes. While less intensive, it encourages awareness and self-guided health improvements.
Cost-Efficient Funding
Both programs can be funded using federal tax savings available through TSG’s supplemental benefit plans, often making them net-cost-free for employers. This approach allows organizations to improve employee health, reduce long-term healthcare expenses, and promote workforce productivity.
